Regular care monitoring provides valuable service - particularly for families managing care at a distance, or relatives, friends and professional attorneys holding formal responsibility.
Through ongoing care and wellbeing reviews, regular visits and check-ins, we assess care quality, identify concerns early, and recommend timely adjustments.
This ensures care remains appropriate, consistent, and aligned with a high quality of life, while providing reassurance that care is being independently monitored.
For those without nearby family, regular visits from a familiar professional can also support wellbeing and reduce loneliness.
Please note:
Visits can be arranged monthly, quarterly, or as required.
An initial review of the Care Plan and Daily Care Records is required to start the service (£350).
Care Adviser visits are charged at £90 per visit.
Registered Nurse (RGN) or Dementia Specialist Nurse visits are charged at £250 per visit.

This service is designed for situations where there are concerns about the quality or suitability of existing care.
We provide an independent review of the care plan, daily care records, and overall care delivery - identifying gaps, inconsistencies, and areas of concern.
Where needed, we support you in addressing these with the appropriate professionals, including care providers, healthcare teams, GPs, and safeguarding services.
We also assess whether appropriate healthcare support and referrals are in place, including access to rehabilitation and specialist services such as physiotherapy, speech and language therapy, dental care, and audiology.
The outcome is clear, structured insight to ensure care remains appropriate and aligned with the individual’s needs.

Depending on complexity of health and care needs Care Plan Reviews may be carried out by Care Advisers, Registered Nurses (RGNs), or Dementia Specialist Nurses.
